We are looking for Adjuvant Melanoma Patients stage III or IV who have not had recurrence of their stage III or IV melanoma to take a 15 minute online survey paying $50.00 to share their story!

Qualifications
Aged 18 years or older
Has had only one complete resection for Stage III or IV melanoma throughout the treatment history, and the surgery or the last surgery, if multiple surgical procedures were involved, was within the past 12 months
Has never had a recurrence of stage III or IV melanoma
